MyDirectives is the first all-digital advance medical directive service, allowing consumers to create, store, update and retrieve their directives at any time from any location. MyDirectives is accessible both via its website, MyDirectives.com and MyDirectives MOBILE™ for iPhone and iPad, as well as can be a module within electronic medical record systems helping hospitals to improve access to directives at the time of a health crisis.〔(【引用サイトリンク】url=http://www.mydirectives.com/ )〕 Users carry wallet cards to make it known they have directives, and periodic emails are sent to those they identify as healthcare agents. Doctors and healthcare providers click a link to access the directives and other documents and messages individuals choose to make accessible. MyDirectives is free to consumers and providers, while health insurance plans pay a nominal fee to provide their beneficiaries the ability to create their directives. Users can log in at any time to update their directives and preferences for treatment and care. People also can upload video messages. The site provides resources encouraging people to discuss their advance care planning wishes with their family while healthy. MyDirectives uses a proprietary universal advance digital directive (uADD) that combines a living will, a medical power of attorney and various other elements (an individual’s thoughts on resuscitation, organ donation, autopsy, and qualitative questions for caregiving) designed to help doctors, nurses, caregivers and family members understand the individual’s thoughts and goals. The uADD is legal in all U.S. states and in countries around the world. The MyDirectives uADD is also the first such platform to give consumers the ability to use audio and video messages to further customize and personalize their advance care plan. MyDirectives allows consumers to attach their Physician Orders for Life-Sustaining Treatment (POLST), Medical Orders for Life-Sustaining Treatment (MOLST), Medical Orders for Scope of Treatment (MOST) or other portable treatment orders to their directive. ==Recognition== MyDirectives is the first advance care platform to receive meaningful-use certification from the U.S. Department of Health and Human Services. MyDirectives is also a supporting organization of the Blue Button program of the U.S. Department of Veterans Affairs and the Centers for Medicare and Medicaid Services to allow downloadable personal health records.〔(【引用サイトリンク】url=http://www.cms.gov/Research-Statistics-Data-and-Systems/Files-for-Order/NonIdentifiableDataFiles/BlueButtonInitiative.html )〕 The service has been endorsed by the Office of Clinical Ethics and Palliative Care at Baylor Health Care System and its director, Dr. Robert Fine, Dr. Michael Wasserman, who served as chair of the American Geriatric Society’s Managed Care Task Force and by former Sen. Bill Frist (R-Tenn.), a heart and lung specialist. Dr. Monica Williams-Murphy, a board certified emergency physician at the University of Alabama Hospital, where she also serves as the medical director of advance care planning and end of life education, called MyDirectives “a pioneer in enabling individuals to articulate our own medical choices.” ''PC Magazine'' listed MyDirectives as one of its "Top 100 Websites of 2013" and MedCity News included MyDirectives as one of 10 startups to offer an early look at 2014 trends. Founded by Scott Brown and Jeff Zucker after both went through experiences with family where medical wishes were not respected due to a lack of advance care planning, (MyDirectives.com ) went live in 2012 and launched MyDirectives MOBILE™ for iPhone and iPad in January 2015. MyDirectives is a service of Dallas, Texas-based ADVault, Inc., a privately held company.〔(【引用サイトリンク】url=http://pevc.dowjones.com/Article?an=DJFVW00020120119e81jajxh5&ReturnUrl=http%3a%2f%2fpevc.dowjones.com%2fArticle%3fan%3dDJFVW00020120119e81jajxh5 )〕 抄文引用元・出典: フリー百科事典『 ウィキペディア(Wikipedia)』 ■ウィキペディアで「MyDirectives」の詳細全文を読む スポンサード リンク
